MyJourney Compass?

Health Information Exchange allows health care professionals and patients to appropriately access and securely share a patient’s vital medical information electronically.   Meaningful use requirements, new payment approaches that stress care coordination, and federal financial incentives are all driving the interest and demand for health information exchange. Working with patients as they use MyJourney Compass to manage their cancer journey helps you as a provider to meet these requirements and participate in a unique project that is first of its kind in the nation.  Below you will find information to help you better understand

MyJourney Compass and how to use it in cooperation with your patients. 

 

MyJourney Compass is a collaborative between Floyd Medical Center, Harbin Clinic, Redmond Regional Medical Center, the Northwest Georgia Regional Cancer Coalition, and Cancer Navigators to enable and empower patients to actively participate in managing their cancer through the use of mobile technology. This project is the first of its kind and is on the cutting edge of health information exchange technology.  

 

MyJourney Compass will provide breast cancer patients in Rome, Georgia with internet enabled tablet devices that integrate electronic health records, Georgia Direct Health Information Exchange, Microsoft Health Vault, and a combination of off the shelf and custom apps to provide a cancer survivorship tool to help them manage their cancer journey.

 

 

MyJourney Compass uses Georgia Direct secure email and Microsoft Health Vault to ensure patient-provider privacy is protected. Microsoft HealthVault is a privacy- and security-enhanced online service designed to put patients in control of their health information. patients will utilize Microsoft to organizae and share their health information with the providers when and wherever they like.  Providers are encouraged to enroll in Georgia Direct in order to communicate with their patients.

 

Patients will also be given a symptom tracker application developed by the Harbin Clinic Oncology staff and made possible by Georgia Tech.  Working with their providers, patients are encouraged to track their symptoms related to pain, fatigue, nausea, sleep, and constipation.  This will allow providers to get an ongoing picture of the patient's health throughout their journey.  This information can be sent to the physician electronically as often as the provider prescribes.

 

MyJourney Compass can be tailored to each cancer patient’s individuality and needs.  It will help them be informed, prepared, and in control as they navigate their treatment.  Mobile technology allow your patients to

communicate with you, organize their care team, manage appointment schedules, and understand their disease and treatment options as they work through tough decisions.
